giving us the following information

a) Course Level
b) Date completed
c) Instructor and Facility
d) Where it took place, and
e) your present mailing address,

and enclosing a cheque for £7.99 payable to IANTD Merchandise Ltd. to cover processing and return postage.

You may also send a passport photo if you want a picture on your card(s), (although this is no longer required).
